For a hospital or local business matter we confirm the operational owner, the source system, its retention and the evidence question before anything is collected. Authority over an employer account does not reach a provider or patient system, so the source register keeps each one on its own row: the employer laptop acquired, the tenant audit export collected, the clinical record waiting on approved access.
The register also shows whether we examined a source directly, received it from another party or found it unavailable, which matters the moment the report is challenged. Patient care changes and vendor approvals stay with the hospital's accountable owners, and the report states the access, clock and retention limits of each source next to the finding it supports.
